Print out 2 copies of the Petition form below . Each of you fill out one form as the witness .
Then witness your friend` signature on your form -- and , of course , vis-versa .
Finally just mail the 2 petitions to the address below by August 15 so we can get them turned in by the August 22 deadline .

EXTRA CREDIT : Since you`ve got these petitions started , how about fleshing out the forms with JohnHancock`s of some others in your neighborhood who also want to have a choice for Congress this fall !
